You are aware that you are using the facilities at your own risk and that if an outbreak occurs, the contact person for your group will be contacted and they are responsible for contacting everyone in the group.

You confirm that you will take full responsibility for any minors that are in your care and acknowledge that you are taking responsibility for any risk they may incur while using the clubhouse facility.

You confirm that you will leave immediately at the end of your scheduled time and take all of your possessions with you.

I confirm that in reserving and booking the Great Room / Multi-Purpose Room that each and every one of my guests attending the reserved time slot will adhere to all Public Health Orders (PHO) in place at the time of the event.

I confirm that each and every one of my guests attending over the age of 12 is double vaccinated and that he/she/they are aware that they will be required to provide proof of vaccination forthwith upon request by a member of the Clubhouse staff or its designated volunteers.

If any such attending guests refuses or fails to produce proof forthwith of being double vaccinated, that guest(s) will be required to leave the building forthwith and not be permitted to re-enter for the balance of the booked function that I have made.

I confirm that each and every one of my guests, regardless of age, will be required to sign in upon entry and provide his/her/their full name and telephone number for the purposes of contact tracing.

I confirm that if any of my guest(s) is not doubled vaccinated who is over the age of 12 and he/she/they refuse to leave the facility forthwith, I acknowledge and am aware that my function shall be shut down immediately. Should my guest(s) refuse to depart forthwith, law enforcement may be called to remove such person(s) and end the booked function immediately.

Great Room will have a MAXIMUM of 50 guests

Multi-Purpose Room will have a MAXIMUM of 18 guests
